Patient-Centered Medical Home (PCMH) Recognition requirements continue to evolve in order to meet the needs of both patients and primary care practices. In July 2025, NCQA announced updates to the PCMH Standards and Guidelines, effective January 1, 2026. The most notable changes are:
- New NYS quality measures
- Increased thresholds for medication reconciliation
- New cadence requirements added for 45 criteria
- Additional evidence upload required for all Annual Reporting criteria
- New documented processes required
- Overall focus on internal communication to improve patient outcomes
