The landscape of healthcare is changing, and we need to be ready to change with it. In July 2024, Centers for Medicare and Medicaid Services (CMS) announced the awardees for a new, innovative, primary care model, Making Care Primary (MCP). The goal of MCP is to improve care for patients by supporting the delivery of advanced primary care services, the backbone of a high performing health system.

MCP includes 133 participants across 8 states, representing 772 primary care practice sites. These practice sites are comprised of Internal Medicine, Family Medicine, and Federally Qualified Health Centers (FQHC). MCP is a 10.5-year multi-payor model that includes 3 participation tracks. MCP is focused on supporting less experienced primary care practices and organizations by helping them build advanced care delivery capabilities, foster coordination with specialty partners and, over time, assume prospective payments and accountability for cost and quality outcomes.*
