Practices making the commitment to become a Patient-Centered Medical Home (PCMH) are embarking on an exciting, enlightening, and an empowering journey. Practices also recognize that this journey can be complex, challenging, and in the end, extremely rewarding. This process may require re-engineering of operating structure, development of policies and procedures, and assessment of current operations. Practices need to be prepared to make changes in workflows in order to successfully achieve and retain PCMH Recognition.

No question, the transformation journey requires significant dedication and sheer determination, but in the process, the practice also achieves many outstanding accomplishments. The National Committee for Quality Assurance (NCQA) reports that as of November 2018, 13,000 physician practices have become PCMH Recognized across the country and the numbers grow daily. This framework provides practices with the tools and processes to provide quality driven patient care with a Patient-Centered approach. Practices that have achieved this distinguished recognition truly exemplify a cohesive team that shares in their passion and commitment to provide the highest level of care while engaging the entire practice team.
